Person centred planning
At Capability we have always taken pride in providing disabled
people with a specialist and individually-tailored service. Now we
are taking that approach one step further through the introduction
of 'person centred planning' to enable people to live the life they
want.
Person Centred Planning places the individual at the centre of all
decisions relating to not only their care and activities but also
their life goals, ambitions and aspirations. The individual chooses
a group of people they consider to be most important in their
lives; be that their friends, family, carers, colleagues or anyone
else. This group then supports the individual to make choices about
and take control over the direction of their life. This ensures
that physical, emotional, social and personal development needs are
met in a truly individualised way.
Many individuals who use Capability's services have already had
direct experience of Person Centred Planning and can see the real
difference it makes to their lives. One such person is
Margaret Anne Howard who has changed the way she receives her
service as a result of her sessions, having opted to take up
additional 1:1 support to attend college one day per
week.
"I found the person centred planning process both positive and
motivating" said Margaret Anne. "Working with people who are close
to me helped me to recognise my skills and gifts and think about
how I could pursue these meaningfully."
Assistant Support Worker, Danielle Morton, felt privileged to be
part of Margaret's person centred planning circle, she said: " I
found the process to be very relaxed and comfortable and a great
way of increasing my understanding of Margaret Anne's needs,
experiences and goals in life.
"On a personal level I have found the experience motivating as
it has raised my awareness of how the support provided by
Capability makes a difference to people's lives. The process
also helps to establish and maintain strong relationships between
staff and the individuals who use our services as we make a
commitment together to helping people achieve their goals."
